This special circumstances application must be signed by the voter. This application can be obtained from your municipal clerk or from the Secretary of State.

Comprehensive information about absentee ballots, including a downloadable request form, can be found at the Department of State’s website – votesPA.com. Your municipal clerk can tell you who is considered an immediate family member under the law.
